The Blended Case Manager position is a full-time position at Aviva Mental Health Services, located in Collingdale, PA. We are seeking a compassionate and organized Blended Case Manager to provide case management and therapeutic support to individuals experiencing mental health challenges. This role combines clinical services with care coordination to help clients achieve stability, independence, and improved quality of life.
Key responsibilities include developing and implementing individualized treatment plans; assisting clients with accessing housing, employment, SSI, SSDI, and other social services; care coordination with other healthcare providers, families, and community resources; monitor client progress and adjust services as needed; participate in team meetings and monthly supervision; maintain a caseload of 30 clients.
Qualifications:
- Associate's degree in Social Work, Psychology, or related field.
- Prior work experience in mental health, case management, or community services.
- Valid driver's license and reliable transportation.
