RHD Critical Time Intervention is a time limited program that works to engage homeless individuals as they transition from an institutional setting, such as a homeless shelter, to a more community based living situation. Acknowledging that the first few months after leaving an institutional setting is critical for long-term adjustment, CTI works with the consumer to create solid, long-term community linkages. Before a consumer first moves to community housing, the CTI team meets with the individual while they are still in the shelter or temporary residence to formulate a treatment plan with specific attention to six areas that facilitate community stability: psychiatric treatment and medication compliance, money management, substance abuse management, housing crisis management, life skills, and family interventions.

The CTI Specialist is responsible for providing hands on case management services to consumers in the field. The CTI Specialist will assist consumers in creating linkages to long term community supports with particular attention to five areas that facilitate community stability: psychiatric treatment and medication compliance, money management, substance abuse management, housing crisis management, and family interventions. The primary goals of the CTI Specialist are to maintain engagement with the CTI consumer, and, to sustain the CTI consumer in a residential setting during the duration of the CTI service. This is essential in order to provide the stability and connectivity necessary to reduce future homeless episodes.


Duties and Responsibilities:
• Assess and evaluate new consumers when assigned to caseload.
• Work in collaboration with other CTI specialists in a team approach to share consumer information, insights, and suggestions.
• Complete consumer Service Plan every 3 months in coordination with Phase transitions or as needed with emphasis on strengths and needs.
• Establish and maintain empathetic, non-judgmental relationships with clients.
• Provide peer mentoring with the focus on accessing and maintaining housing.
• Participate in CTI Team meetings, county homeless meetings, and all other appropriate meetings/training outside of the agency.
• Participate in individual supervision as required by Director.
• Make routine home visits and accompany consumers in the community.
• Gain access to services with aggressive and creative attempts to help consumers obtain resources that are needed. This includes strong role as a consumer advocate.
• Monitor service delivery by maintaining regular contact with consumers and service providers/programs involved with the client.
• Work in conjunction with CHOC shelter staff to insure appropriate service delivery for the consumers.
• Monitor consumers on a regular basis as determined by consumer’s Phase of service to note any changes that may impact his/her treatment/recovery.
• Maintain clear and concise documentation of all client contacts/referrals and ensure quarterly forms are completed accurately and filed in consumer’s chart.
• Complete all required paperwork, i.e., Service Documentation, Consumer Release Forms, etc. within timeframe designated by Director.
• Provide on-call emergency service at least 1 week/month.
• Other related duties as assigned.

Documentation Responsibilities:
• Learn and use the Montgomery County Homeless Management Information System (HMIS).
• Learn and apply consumers for benefits through COMPASS.
• Complete required all documentation for data collection and billing requirements (28 hours per week) established by the licensing and funding agencies of CTI.

Qualifications:
• Bachelor’s degree in social work, psychology, rehabilitation, activity therapies, counseling or education with at least 3 years mental health/D&A experience; experience working with a homeless population and providing outreach to homeless is preferred.
• Valid Driver’s License required and clean driving record
• Access to reliable transportation.
• CPR/1st Aid Certifications
• The ability to interact professionally with clients, their family members, and other service providers is essential. Must be able to work with a team and demonstrate flexibility. Must be willing to travel to the homes of clients, designated facilities, and known street locations where people can be found.

• Demonstrated success in managing and developing staff, consultants, partners, and advocates
• Excellent relationship-building experience, with superb communication skills including the ability to tailor and convey ideas and positions as a spokesperson for charter schools to a wide range of constituent groups
• Professional maturity and sophistication to act as the public face of PCCS
• Knowledge of issues and challenges facing education reform and school choice, the national charter school landscape, and K-12 education circles in Pennsylvania preferred
